Verizon phone service.
1st I tried on my own
2nd I asked Verizon tech for help we were not successful. She suggested I go to a Verizon store.
3rd. The Verizon rep finally got it after 45 minutes of trying.
I'm not sure who is at fault here but it was not an easy process.

For those paperwork to be loaded, if you have Facebook send a picture of your paperwork to your Facebook message. Than when it ask you to upload your paperwork go to Facebook message.
