Don't do it! I bought vouchers 2 years ago, have tried numerous times to book, even with the help of Bianca, and asked for a refund numerous times since i have still yet to get service or the refund. BEWARE!
No, the form does not offer many booking preferences. You get to pick One date, One time, and then wait up to the day before to see if anyone will come.
Also be aware that there is no tax on massage services in Florida, so what's that charge for?
If you are in a situation like mine I got my voucher through Groupon and I had not problems getting my money back when I finally gave up on getting an appointment set up.
