Your staff was tremendous. The only issue I had was with the UPS delivery which was delivered to the neighbor and had been opened by them before I was made aware that the package was not delivered to my house.

I knew that I would have issues because my birth certificate does not reflect the name I've used all of my life. In the late 1970s it was legal to change your name or drop your first name upon marriage. However in the 1970s, there was no record of the legal name change for a woman, only the husband was required to sign the marriage certificate after the ceremony. If the wife chose to go from "Suzy Jane Smith" to "Jane Smith Jones" upon marriage, the only documentation of the change would be a social security card in the name of Jane Jones or purchase of property by Mr. & Mrs. Jones. Rush My Passport's customer Service was very helpful in recommending the documentation I needed to support my name change and recommended that I send the passport application with a cover letter explaining the purpose of the additional documents. Because of their foresight, I had no problems receiving my passport.
