It's a scam! They will only e-mail you the form to send which is free on each states web site.

I contacted my credit card company before the transaction posted to flag it. Once it posts, I'm going to ask the credit card company to dispute the charge.

You will never get one if you are waiting for Onlinevitalus.com to get it for you. They should have sent you an email telling you to print the form and mail it to the state along with the states processing fee. They take $47.00 for filling out a piece of paper you can do for free.
