Well, I did fill out the form online, but a PDF file to print and mail is NOT what I expected. And I couldn't even edit it.
The ONLY good part about US-forms.com is that they guaranty a refund if you aren't satified, and I was NOT satisfiied. They say they processed the refund. With that, I can make certain when I get my credit card statement.

There, I found very clear instructions for gathering the required supporting documents, and an online form to fill out AND SUBMIT that was very, very easy. Unlike the scam site, the only fees were the actual filing fees.
