My primary rhinoplasty, revision rhinoplasty and filler experience with Dr Zacharia

I would like to preface by clarifying that the following is a factual account of my experience and is not intended to defame the character or practice of Dr Zacharia. In 2021, I sought a consultation with Dr Zacharia to reduce the size of my nose and to slightly raise the tip. Subsequently, I sought a second consultation with Dr Zacharia to inquire about the necessity of waiting for threads to naturally dissolve in my nose before proceeding with the surgery, as advised by my cosmetic injector. During this consultation, I expressed my desire to make additional changes with the morph, including a very slight raising of the tip and eliminating any visible nostril show.

Prior to my primary rhinoplasty, I reiterated my desired outcomes to Dr Zacharia, emphasising that I wanted only a very slight raising of the tip and no visible nostrils. However, upon receiving the results of my surgery, I was shocked and extremely upset to find that my nose had been raised significantly higher than desired, my columella was removed and retracted, and that my nostrils were now extremely visible from the front. At my six-week post-op appointment, Dr Zacharia expressed uncertainty about whether the threads had caused these complications. Despite these concerns, Dr Zacharia proceeded with a revision and promised to lower my nose and reduce nostril visibility and to fix my retracted columella by adding ear cartilage to the columella.

Unfortunately, this revision did not yield the desired results (my columella is still retracted, I have an indent on my columella which is visible on frontal view, my nostrils are uneven, I have bumps on my nostrils that have lasted over a year and excessive nostril show), and Dr Zacharia subsequently resorted to injecting filler into my columella. However, this intervention did not resolve the issue, as a visible dent remained in the columella. Dr Zacharia also removed cartilage from my nose tip, despite the fact that we had not discussed this prior to the surgery. Ultimately, the results of both my primary and revision rhinoplasties left me feeling extremely disappointed and unhappy with my appearance.

I have since sought the opinions of other surgeons, all of whom have recommended further revision to address the remaining issues with my nose. Additionally, I experienced a concerning incident when Dr Zacharia injected filler into my columella, during which I experienced a vascular occlusion. Despite expressing my concerns to Dr Zacharia about the unusual symptoms I was experiencing, he dismissed my concerns and advised me to "monitor it". I was also advised to call back the next morning, as the office was only open for a limited time that day. I was dismayed to learn that Dr Zacharia would not have administered hyalase, the recommended treatment for a vascular occlusion, had I been not been in Sydney at the time.

I would like to reiterate that this account is not intended to disparage Dr Zacharia or his practice, but rather to provide an accurate record of my experiences and concerns.
