I wanted to give you a brief update on my experience in submitting the application in case you here from others. It was a complete $#*! show LOL. The application on the NYS website is outdated and not permitted. All the forms have been redone. Beside you license you must have a current passport or original birth certificate. You must still list proper cause for issuance of a permit. You must list all addresses for the last five years, an authorization for release of information. The character references must live in the same county as the permit. New form for the character reference which must be notarized and they must also sign the application. Even if you submitted the application today and it was processed the applicant will fall into the new law because it is the date of the permit not the date of the application so we are required to take 16 hours of classroom as well as 2 hours on the range. The applicant is also required to list all social media accounts. I have all the documents required if you want a copy just ask and I can email them over.
