What is the best way to provide customer feedback and talk to someone who can make a decision?

You can call and make a complaint. Someone with absolutely no authority to act upon your probably sincere desire for better service will transcribe your words and sent them off where they may or may not be reviewed. The only real ability to have to connect with someone who can make a "decision" is if you're negotiating the terms of a service arrangement, in which case just play hardball, or keep calling back in until you get what you want.
