My son attended the Carlsbad camp 7/1/16 - 8/4/16. I'm trying to submit for dependent care reimbursement. In order to do this I need an invoice with the following information:
Name of service provider
Specific dates of service
Name of dependent receiving services
Description of service
Purchase amount for each service
Total purchase amount
