They don't pay you the profits you make on a demo account yeah, that's the main difference!
Let me clear things out for you.
With a demo account, you trade virtual money, and its purpose is to learn trading in a risk-free environment.
A live account is the place where you actually trade real money, and make real profits. You can withdraw the profits to your bank account, but also you can lose money.
A demo account is free, while for a live account, you need to deposit funds to trade with.
